Risk Assessment Form

Submission DUE Friday, September 8

  • Required in order to participate in the event.

  • If you are bringing in a machine, chemical, or require rigging, please have all drawings and data sheets available prior to completing the questionnaire.

  • If you are bringing in a 3rd party booth/stand builder, please have all booth drawings, 3rd party company information and proof of insurance available prior to completing the questionnaire. It is also required to complete Shepard's forms located within the online ordering portal.
